Preparing for a Visit

• A detailed Medical History is required for all new patients. Please complete our New Patient Medical History Form and bring it with you on your first visit. You may complete the form online and print the completed form. (You cannot save the completed form online.) Alternatively, you can print the form and complete it by hand.
